This state-of-the-art Center located at the Eastside Heritage Center on 2160 Fountain Drive in Snellville, Georgia features 20 private rooms and is focused on helping you cope with the hurdles of recovery while preparing you for return to your life, work, family, active retirement and independent living.
We are dedicated to providing the care necessary to meet your medical, physical, psychological, and social needs in an environment that promotes wellness, confidence, and independence. Our program provides a place where those who have suffered disabling illness or injury begin to rebuild and enrich their lives. Along with the support of a family and/or friend, the patient learns the necessary skills to expedite discharge.
Special features include a high-tech media theatre and entertainment room, barber shop/beauty salon, state-of-the-art therapy gym, outdoor courtyard, and social events like cook-outs, game night and movie night. For the convenience of our patients, the Center provides a furnished “apartment” and “grocery store” where patients may practice daily living activities prior to discharge.
Our Rehabilitation Staff consists of the following professionals:

- Speech Therapists
- Case Managers/Social Workers
- Neuropsychologist
Our team of rehabilitation professionals provides services to patients experiencing:
- Limitations in self-care
- Impaired mobility, balance, and coordination
- Limitations in strength and range of motion
- Cognitive and perceptual deficits
- Speech and communication problems
- Swallowing disorders
Common rehabilitation diagnoses are:
- Stroke
- Hip Fracture
- Polyarthritis (including rheumatoid arathritis and degenerative joint disease)
- Major multiple trauma
- Amputation
- Congenital deformity
- Brain and spinal cord injury
- Neurological disorders including multiple sclerosis, motor neuron diseases, polyneuropathy, Parkinson’s disease, and muscular dystrophy

Saturday and Sunday 1:00 p.m. to 8:00 p.m.
